A diastema is the medical term for a gap between two teeth. This condition commonly occurs between the top two front teeth known as the upper incisors. The gap between these teeth can occur for several reasons.

A diastema itself is by no means a harmful medical condition. Generally, it is simply cosmetic, and correcting it would depend on the individuals personal perception of it.

There are a few main procedures that are used to close the gap. All can guarantee results, but deciding between them depends on the personal preference of the individual.

Braces are a great way to take care of any orthodontic problem. The procedure will leave the natural teeth intact, and merely rearrange them as needed. However, braces are required on the whole row of teeth, to ensure they all move into position. It also can be a lengthy process, and is by no means a quick and easy fix.

If your upper incisors are small, you can widen the tooth to eliminate gap between them. This can be done with a bonding, veneers, or crowns. Bonding works by putting composite filling material between the teeth to extend them. Veneers are a thin layer of strong porcelain material which covers the front of your tooth to widen them. Crowns cap the entire tooth and can be fitted at any needed size.

When teeth have existing crowns, or when they have large fillings or cavities that take up a large portion of the existing tooth, veneers or bonding may not be a good option to restore front teeth. Crowns cover the entire tooth and can provide strength, longevity and excellent cosmetic results if done correctly. 

The crowns below were replaced with new, highly esthetic porcelain. It is very important to pay special attention to your bite and the way your teeth function to ensure predictability. You can see in the photos below the bright new teeth compared to the old crowns with dark edges. This work was completed in our office by Dr. Workman. He treats each case individually and custom fabricates crowns that look like no dental work had even been done.

Tooth Replacement with Dental Implants

In the past, the traditional way to replace all of your teeth was to use full dentures. Full dentures can provide a very aesthetic outcome as well as allowing you to chew certain foods that you would otherwise not be able to chew. There are however several limitations with traditional dentures.  Traditional dentures are not fixed to the underlying bone and therefore have the tendency to move around, make clicking noises or even fall out of your mouth. Because they are not firmly secured to the underlying bone, chewing food can be very challenging. Most importantly, over time bone loss continues to occur. This can affect your ability to chew and the fit of the dentures. With bone loss occurring in the jaw, long-term denture wearers often look much older.

With dental implants we can significantly improve the problems with traditional dentures. Implants can fix the dentures to the bone though small attachments on the implants. Implant supported dentures work more like natural teeth by making it easier to chew, speak and feel more confident. Also, with dental implants you no longer have to worry about losing bone. Implants simulate health of the bone allowing you to have a natural, healthy and comfortable outcome.

Implant Supported Dentures - Broomfield

An implant-supported denture is a type of denture that is supported by and attached to implants. A regular denture rests on the gums, and is not supported by implants.
An implant-supported denture is used when a person doesn't have any teeth, but has enough bone to support implants. An implant-supported denture has special attachments that snap onto attachments on the implants.

There are two types of implant-supported dentures: bar-retained and ball-retained. In both cases, the denture will be made of an acrylic base that will look like gums. Porcelain or acrylic teeth that look like natural teeth are attached to the base. Both types of dentures need at least two implants for support.

• Bar-retained dentures — A thin metal bar that follows the curve of your jaw is attached to two to five implants that have been placed in your jawbone. Clips or other types of attachments are fitted to the bar, the denture or both. The denture fits over the bar and is securely clipped into place by the attachments.
• Ball-retained dentures (stud-attachment dentures) — Each implant in the jawbone holds a metal attachment that fits into another attachment on the denture. In most cases, the attachments on the implants are ball-shaped ("male" attachments), and they fit into sockets ("female" attachments) on the denture.

Your implant-supported denture will be more stable than a regular denture. You will find it easier to speak and you won't have to worry about the denture becoming loose or falling out of your mouth. You generally will be able to eat foods you could not eat before.

If you have an implant-supported denture in your upper jaw, it can be made to cover less of your palate (roof of your mouth) than a regular denture. That's because the implants are holding it in place instead of the suction created between the full denture and your palate.

Implant supported dentures are an ideal way to restore esthetics, comfort and function.

How long does it take?

Fitting a crown requires at least two visits to our office. Initially, we will remove decay, shape the tooth, and fit it with a temporary crown of either plastic or metal.

On the subsequent visit, we will remove the temporary crown, and then fit and adjust the final crown. Finally, we will cement the crown into place and you'll have a new beautiful tooth.

What are the capabilities of crowns?

Crown and bridgework is a very reliable solution for major dental problems caused by accidents, diseases, or wear and tear. We can generally correct major problems with crowns or bridgework. The material we use is either high-grade porcelain or porcelain bonded to gold or metal. A higher strength of the porcelain and gold materials is recommended to treat serious problems. For lost or broken teeth, crowns and/or bridges offer a long-term solution.

Many people have unexplained pain from fillings in back teeth; this is usually due to hairline cracks in the chewing part of the tooth. Placing crowns on these teeth relieves the pain and allows full dental function to return. In front teeth, older fillings can weaken the teeth and cause appearance problems due to staining or chipping. Porcelain crowns and bridges are suitable in cases where porcelain veneers are not. In teeth with root canal fillings, crowns can prevent breakage.

If you have old silver fillings that need replacement, we can restore those teeth with tooth colored composite fillings. With these fillings, we are able to match existing tooth shade so the filling is not detectable. Even though your silver fillings may not be sensitive or sore, if they are leaking, you may need to replace them in order to avoid cavities beneath the silver. The filling below had a cavity under the silver which was not visible when we started the procedure. We removed the cavity and restored the tooth to it's natural appearance.

Kois Center

Last week I was in Seattle at the Kois Center. I spent three full days of learning new techniques and procedures. I studied the key parameters in the decision making processes in dentistry - gums and bone, teeth, jaw and bite and esthetics. Through evaluation in these important areas we established guidelines to produce more predictable results each day in the office.

The learning center was very modern, with windows looking out onto Lake Union in Seattle.

Each dentist in the group brought a dental case to present and review with the other dentists attending. We used the treatment planning techniques learned in the course to plan ideal treatment for each case.

Unlike most days in Seattle, we got some sun and blue skies! It was beautiful.
I am excited to contiue learning at the Kois Center. Other courses include implants, esthetics, occlusion, and dentures. My next visit will be November 2011.
